- Your health history, current medications and past injuries
- The symptoms that bring you in and how they affect your day
- Consent and office policies
Everything you share is kept private and is used only to plan your care.
What to Bring to Your First Visit
- Your completed new patient paperwork
- A photo ID and your insurance card
- A list of medications and supplements
- Any recent imaging reports or records related to your pain, if you have them
- Comfortable clothing that lets you move easily
